Advocate Medical Group (now Advocate Health) – Condell MOB & Ambulatory Facility

The program for the Advocate Medical Group (now Advocate Health) Condell clinic facility was developed in conjunction with the strategic planning process. It identified the consolidation and growth of smaller medical practice sites into more highly-visible, larger facilities that will enhance service to patients, provide operational efficiencies, and promote organizational growth. 

The new facility consolidates current providers into a single location in close proximity to the Condell Medical Center. The clinic is anticipated to house all clinical functions and most support functions. 

Pre-Design of a standardized “Core Module” was a part of the strategic planning process. The Core Module is a standardized layout to be used as the typical floor plan for planning and design of outpatient clinics within Advocate projects. Additionally, the clinic will follow Advocate branding standards for signage, interiors, systems and room standards.

Project Highlights

  • The Care 360 Integrated Ambulatory Care Model provides guidance for development of outpatient clinics throughout the Advocate system.
  • The facility includes a small gallery space that features the work of local artists and students as part of a move to connect health services with the local community.
  • The AOC IPD team has delivered more than 26 projects since its formation.
